Below are a list of sources that may be
helpful with your name change.
Social Security

Form SS-5
contains detailed instructions on it's use for both name and address change.
Fill out the form and mail it along with a certified true copy of your
divorce record showing both your old and new name. You may
contact the clerk of the family court where you were divorced.

Obtaining a certified copy of
your Divorce Decree - visit the South Carolina Department of Health and Environmental Control website for request forms for all vital
records held in South Carolina.
Drivers License
- South Carolina Department of
Motor Vehicles - fill out form 4057 and either mail it back to the
Department of Motor Vehicles, or hand deliver to the nearest DMV branch
office. You may also want to obtain a change of name on your car's title, this
can be done by filling out form 4057 as well as form 400 which is available

Passport
Notary - for South Carolina Notaries
- Any notary public whose name is legally changed during his term of office
may apply to the Secretary of State in such manner as may be prescribed by
him, and the Secretary of State may change the name of the notary upon
proper application and upon payment of a fee of ten dollars. The term
expires at the same time as the original term.
US Post Office
- The US Post Office does not require notification of a name change.
However, if you have moved you can fill out a Change
of Address form, which will ensure that mail in your old name which may
go to an old address will be forwarded to your new name at your new
address.
Other agencies you may want to
notify may require no "Official" form. The "Notice
of Client Name Change" form can be used to notify the following
agencies:
